All patient forms will be completed digitally via our secure patient portal on Clinicmaster. An email will be sent to you once you are booked over the phone and once completed, you simply send it back to us.

Prepping for your initial appointment
- Please follow our Covid-19 Appointment Protocol per the website under COVID-19 Update.
- Arrive as close to your appointment time as possible.
- Please wear appropriate clothing to expose the injured area of your body.
- Please bring any documentation regarding your injury or surgery ie. test results, referrals, specific post-op orders
- We will need a copy of your insurance card as well.
- It’s a good idea to make a list of questions that you have to discuss with your therapist.
What your appointment will look like:
Your initial assessment will be with a registered physiotherapist, or other professional depending on the service needed, and will take approximately one hour and include a treatment.
- Your physiotherapist will meet with you in a private room to discuss your symptoms and record your full health history. Try to be as honest as you can; open communication helps us to understand your problem better!
- Observation of your posture, gait and movement and measurements of your movement, strength and coordination will be taken. Our goal is to help you, so please make sure that you communicate with your therapist if anything hurts or bothers you during the examination!
- Our physiotherapist will recommend and prepare a customized course of treatment to ensure the quickest recovery and the best results. Some treatments will be performed on the first day to get you started on the road to recovery.
- After your therapist has finished, you will schedule a series of visits at the front desk. The amount of therapy needed will be dependent upon the advice of your therapist, and will also be discussed with you at your first visit.
We offer a lot of convenient appointment times, but prime times can be busy, so it’s best to book ahead as much as possible.
